– MODEL RELEASES for your Photo Shoot (DOWNLOAD) –

Please be so kind as to click here (for our Standard Model Release) or click here (if 1 or more CHILDREN are part of your Photo Shoot) to download the Standard Model Release that everyone Photographed, Video Recorded, or Filmed by Pascal Monmoine (or by any of his assistants or anyone on his team) must fill out.

Please be so kind as to Print it, Fill it out, Date it, Time it and Sign it… and please MAIL IT back to us at least 3 WEEKS PRIOR to your Photo or Video/Film Shoot with Pascal (if we have not received it within that time, the date booked for your shoot will unfortunately have to be canceled and assigned to one of our clients on waiting list, and your shoot will have to be rescheduled at a later date).  Thank you so much for your understanding.

P.S.  If applicable, please feel free to cross out the standard fee already in this Agreement, and replace it with the fee that you and Pascal Monmoine have agreed upon for your personal Photo/Video Shoot.
